What OSHA Requires.
OSHA's Respiratory Protection standard (29 CFR 1910.134) mandates that any employee required to wear a respirator must first receive medical clearance from a Physician or Licensed Health Care Professional (PLHCP). This applies to filtering facepieces, half and full-facepiece respirators, powered air-purifying units, and supplied-air equipment.
The mandatory Appendix C questionnaire must be completed annually, administered confidentially, and reviewed directly by the PLHCP. Your employer is prohibited from viewing your responses.
If an employee answers positively to any of questions 1 through 8 in Part A Section 2, a follow-up medical examination is required before clearance can be granted. Annual fit testing must be paired with the questionnaire to fully satisfy OSHA's respiratory protection program requirements.
